How do I change my name on present N.C. unemployment benefits record?

Question

I applied through social security to have my name returned to my maiden name since I am divorced and it is finalized and I obtained a new drivers license.  Now I need to make the necessary changes on my unemployment.  I am in the process of changing all my records with all institutions that I have responsibility.

If you go to your state Unemployment website you will find request form to fill in order to change the name and contact details you can use that form and apply for the changes to be done........
