I currently have FMLA/ADA through my employer. However, the process was made unnecessarily difficult and even though it is now in place I am still being contacted and asked repeatedly for information I have provided 3 and 4 times already.  Once I was threatened by the head of the company HR department that my benefits would be revoked if I did not turn in a certain packet of paperwork.  Again, this was one that I'd given them already, and more than once.

I have been made to explain my situation multiple times for various reasons to no less than 4 supervisors. Always with co workers close enough to hear.
  
And while there is nothing I can point to as an outright violation, there is a big deal made of my absences even though they are protected by my ADA status.  

I feel this is at the very least retaliation if not harassment and resulting in the worsening of my condition.

Does this qualify as a reason to quit and leave me elligeable to collet unemployment?

You may want to consult an attorney.  Also, you may try going to the head of HR and respectfully explain your difficulties with how you have been treated.  Talking to your doctor is another thing I would consider.  At all times try to preserve a good working relationship.  It is impossible for me to assure you as to the outcome if you finally quit because of this - but by trying these things, you may have a better idea of the answer yourself.  You can always call the unemployment office too.
